C/ME Program - New Coroner, Medical Examiner Profile Request	USE: Requesting new person profiles for personnel registering for training or applying for a certification from the Coroner/Medical Examiner Program. IMPORTANT: It is required to send a Notice of Hire to the Coroner/Medical Examiner Program within 15 days of hire per WAC 139.27.020. Email the form to cme@cjtc.wa.gov. Firearms Certificate Program New Company Profile	USE: A private security, private investigation and bail bond recovery companies that are NOT listed in the WSCJTC Acadis Portal. LEOSA Profile Request	WSCJTC does not issue LEOSA credentials. US Code 926 C says you must have a photo ID issued by the agency you separated from stating you meet the requirements of LEOSA. An Acadis profile is required. Use this webform to create your profile and Acadis login. This form is separate from the application process. New Civilian , Private Security or Non-Certified Portal Account Request	USE: Request a profile for civilian, private security, or non-mandated personnel. This form cannot be used for certified officers, certified corrections, reserves, or limited authority personnel at WA Liquor & Cannabis, WA OIC, WA Parks, & WA Gambling Examples: Out-of-State Personnel, Court personnel, private security/investigators, bail bonds, federal employees, fire fighters, etc.
New Organization Entry into WSCJTC Acadis Portal Request	USE: Requesting a new organization entry into the WSCJTC Acadis Portal. As examples: municipal and district courts, mental health agencies, social workers, juvenile courts/detention, fire departments, animal control agencies, school districts, tribal agencies and other NON-LAW ENFORCEMENT/CORRECTIONS agencies. It can only be submitted by authorized training or HR personnel.
WSCJTC Commission Check Webform	WSCJTC Commission Check is available for agencies to request a verification of certification and standings in Washington State.
